Thank you for your interest in our 2010 Vacation Bible School.
In the application form below there are two sections:
Top Section: If you are a parent or legal guardian enter your contact information.
Larger Comment Section: Supply the following information for each child who is planning to attend in the comments field.
1. The child's full name.
2. The child's class grade level.
3. The child's home address.
4. Your Emergency phone #.
5. Does this child have any physical limitations or allergies?
Important Notice: Be aware that we will be taking photos and videos of all of our participants and during the course of our VBS week so that we can share the memories of this special week with others. For the safety of our children we do not publish the full names and addresses of those who participate. If you have any questions with regard to this policy please contact our VBS staff before you fill in the information form.
